Summary of referral criteria
The following requirements must be fulfilled when making a referral.
- Patient Eligibility: The patient must satisfy eligibility criteria for public oral health services.
- Patient Consent: The decision to refer must be agreed to by the patient or if under 18, by the parent or legal guardian.
- Fees: All patients should understand that fees will be charged for all appointments, including screening visits that may or may not lead to treatment.
- Continuity of Care: The referring clinician or clinic must have accepted ongoing responsibility for the patient’s general dental management and continuity of care.
- Specific Need Identified: Referrals should be part of a treatment plan and be a specific request for a type of specialist service.
- Referral Form: Referral is to be made on the Specialist Services Referral Form (Attachment 2 in above procedure)
- Specialist Unit Referral Criteria Satisfied: The patient satisfies the stipulated referral criteria for that specialty.
- Relevant Information and Diagnostic Tests Provided: Additional information requirements are stipulated in the specialist unit referral criteria and must accompany the referral.
