Referrals

Oral health professionals who wish to refer a patient to a specialist service within The Royal Dental Hospital of Melbourne must complete the relevant referral form and submit it to:

Summary of referral criteria

The following requirements must be fulfilled when making a referral.

  • Patient Eligibility: The patient must satisfy eligibility criteria for public oral health services.
     
  • Patient Consent: The decision to refer must be agreed to by the patient or if under 18, by the parent or legal guardian.
     
  • Fees: All patients should understand that fees will be charged for all appointments, including screening visits that may or may not lead to treatment.
     
  • Continuity of Care: The referring clinician or clinic must have accepted ongoing responsibility for the patient’s general dental management and continuity of care.
     
  • Specific Need Identified: Referrals should be part of a treatment plan and be a specific request for a type of specialist service.
     
  • Referral Form: Referral is to be made on the Specialist Services Referral Form (Attachment 2 in above procedure)
     
  • Specialist Unit Referral Criteria Satisfied: The patient satisfies the stipulated referral criteria for that specialty.
     
  • Relevant Information and Diagnostic Tests Provided: Additional information requirements are stipulated in the specialist unit referral criteria and must accompany the referral.
