What do I need to send?
Download the Common Application Form from http://www.health.vic.gov.au/ethics/single/common_app_form.htm.
Send an original single-sided signed hard copy and an electronic copy of the completed application form plus any required attachments to the Secretary of the Human Research Ethics Committee (HREC).
Contact the Secretary on (03) 9341 1137 or ethics@dhsv.org.au to inform him or her of your submission.
Note: To ensure you are using the latest version of the forms, please download required documents directly from the website each time you prepare a new application.
When do I need to send it by?
You must send your application to the Secretary at least three weeks before the Committee meeting at which you wish it to be considered. Check the schedule (PDF, 19 KB) for Committee meeting dates.
The number of new applications accepted for each meeting is limited and applications are accepted on a ‘first in, first served’ basis. It is highly recommended that researchers submit applications as early as possible prior to the meeting at which they wish their application to be considered.
