What do I need to send?
Send an original single-sided signed hard copy and an electronic copy of the completed application form plus any required attachments to the Secretary of the Human Research Ethics Committee (HREC).
Contact the Secretary on (03) 9341 1328 or ethics@dhsv.org.au to inform him or her of your submission.
Note: To ensure you are using the latest version of the forms, please download required documents directly from the website each time you prepare a new application.
When do I need to send it by?
To enable review at the next scheduled meeting, projects must be submitted to the Secretary at least 3 weeks prior to the HREC meeting date.
